29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. At least one 1 (1) employee was exposed to struck-by hazards from falling tools/material from above where an employee worked from an aerial lift approximately 12 feet above ground. On or about March 20, 2023 and at times prior, employees were working on in aerial lift trucks repairing the signs to building approximately 12 ft from the ground while other employees walked below them without protective helmets.

29 CFR 1926.453(b)(2)(iv):Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. On or about March 20, 2023 and at times prior, employees were working on in aerial lift trucks repairing the signs to building approximately 12 ft from the ground below. Employees were exposed to a fall of approximately 12 feet to the concrete below when standing on the midrails.
