29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about March 22, 2023, an employee was exposed to machine hazards associated with moving parts and in-going nip points during service and maintenance of the robot and the turntable located inside the PerformArc Robotic Welding Area. The employer failed to develop, document, and utilize machine specific procedures for the PerformArc Robotic Welding Area to protect employees from machine servicing hazards. As a result, hazardous energies including mechanical, pneumatic, and electrical were not isolated, exposing employee to struck by hazards associated with the rotating movements the of metal robotic arm and caught in hazards associated with the traversing action of metal turntable.

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 22, 2023, an employee was exposed to machine hazards associated with moving parts and in-going nip points during service and maintenance of the robot and the turning table located inside the PerformArc Robotic Welding Area. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. As a result, hazardous energies including mechanical, pneumatic, and electrical were not isolated, exposing employee to struck by hazards associated with the rotating movements the of metal robotic arm and caught in hazards associated with the traversing action of metal turntable.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about March 22, 2023, the employer did not ensure that the drill press (Clausing 20) was effectively guarded to protect employee(s) at the point of operation and to prevent employee(s) from reaching into hazardous area. Employee(s) operating the drill press are exposed to moving mechanical parts and other hazardous areas not effectively guarded such as but not limited to spinning motion of the drill bit, spindle assembly and the drill chuck. (b) On or about March 22, 2023, the employer did not ensure that the PerformArc Robotic Welding Area was effectively guarded to prevent employees from entering the hazardous area. Employee(s) operating inside the Robotic weld area were exposed to struck by hazards associated with the rotating movements the of metal robotic arm and caught in hazards associated with the traversing action of metal turntable.
