29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about March 22, 2023, employees working in the 4th Hand Area were exposed to crushing hazards while operating the Caterpillar Forklift Model GP36N equipped with a rotating clamp attachment because the employer modified the attachment by mounting a side rail without obtaining manufacturer written approval.
