29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. 417 Main Street, East Hartford, CT, office building roofing project: On or about March 22, 2023, the employer did not ensure appropriate fall protection was implemented for employees while performing commercial roof repair activities on a flat roof that was less than 50 feet wide. The employees were exposed to a fall of approximately 14 feet to the ground.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: 417 Main Street, East Hartford, CT, commercial building roofing project: On or about, March 22, 2023, employees were permitted to access the upper flat roof from the lower level flat roof at the entrance by stepping a distance of approximately 5 feet without a ladder exposing them to fall hazards.
