Grinding Area:  On, or about, March 24, 2023, the hood guard provided on the Reid surface grinder did not cover the spindle, nut and flange projections.  B.	Grinding Area:  On, or about, March 24, 2023, the crescent shaped guard provided on the Brown & Sharpe No. 13 OD grinder did not cover the spindle, nut and flange projects and was poorly adjusted.

Grinding Area:  On, or about, March 24, 2023, a surface mount box designed for permanent mounting was used in a pendant drop assembly to energize a switch controlled quad box mounted on the side of the machine.
