29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: On or about March 20, 2023 employees were exposed to a fall of approximately 19 feet while performing roofing activities on a residential home located at 2305 Pebble Beach Drive, Massillon, Ohio 44646. The employees were working on all areas of the roof and the employer did not ensure they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) On or about March 20, 2023 employees were exposed to a fall of approximately 9 feet while performing roofing activities on a residential home located at 2305 Pebble Beach Drive, Massillon, Ohio 44646. The employees were using an extension ladder to access the roof and the employer did not ensure it extended three feet above the landing area.
