29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to injury and illness. On or about November 25, 2024, the employer did not ensure employees engaged in installing shingles on a residential roof located at 591 Daleview DR, Aurora Ohio 44202, 10.1 feet above the ground were protected by protected by guardrail systems, safety net systems, and or personal fall arrest system. The owner/foreman was on site and working along side the exposed employees as they worked 10.1 feet above the ground exposed to fall hazards.

Employees were using a portable ladder to access the roof, and the employer did not ensure it extended at least 03 feet above the upper landing surface.
