29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a. On or about March 28, 2023, at the above addressed jobsite, an employee was exposed to falls while performing masonry operations from a tubular welded frame scaffold, eleven (11) feet eight (8) inches above the ground, that did not have the working level fully planked.  b. On or about March 28, 2023, at the above addressed jobsite, an employee was exposed to falls while performing masonry operations from a tubular welded frame scaffold, fifteen (15) feet above the   ground, that did not have the working level fully planked.

29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Note:  Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift.  a) On or about March 28, 2023, at the above addressed jobsite, an employee mixing mortar and moving material on a wood pallet was elevated by a Lull 644E-42 Telehandler forklift, that was not secured to forks, thereby exposing the employee to approximately fifteen (15) feet fall hazard.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about March 28, 2023, at the above addressed jobsite, employees performing masonry operations on a house from a  scaffold, at fifteen (15) feet above the ground, were exposed to fall hazards due to the lack of guardrails or wearing a personal fall arrest system (PFAS).  b. On or about March 28, 2023, at the above addressed jobsite, employees performing masonry operations on a house from a  scaffold, at eleven (11) feet eight (8) inches above the ground, were exposed to fall hazards due to the lack of guardrails or wearing a personal fall arrest system (PFAS).

On or about March 28, 2023, at the above addressed jobsite, an employee performing residential masonry operations, thirteen (13) feet three (3) inches above the ground, was not protected from falls, thereby exposing the employees to fall hazards.
