29 CFR  1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  (a) On or about March 29, 2023, an employee operated a Genie GTH-1056 off-road forklift without being trained in safe operation of the forklift at the jobsite located at 9501 W Loomis Road, Franklin, WI 53132. This exposed employees to a crushing hazard while operator was picking up pallets of shingles and booming the shingles to the roof.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) On or about March 29, 2023, employees conducting roofing work on the southwest and northwest sections of existing apartment building roof, with a 4:12 pitch were not using fall protection while installing new roof. This exposed employees to a fall hazard of approximately 28 feet to the ground below.   Vulkan Roofing & Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1480067, citation number 2, item number 1 and was affirmed as a final order on October 20, 2020, with respect to a workplace located at 738 Washington St. in Park Ridge, IL 60089.   Vulkan Roofing & Carpentry, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1317751, citation number 1, item number 2 and was affirmed as a final order on November 7, 2018, with respect to a workplace located at Grandview Townhomes 1828 Elizabeth Circle in Sycamore, IL 60178.
