29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a)  On or about April 26, 2023, located at 9501 W Loomis Road, Franklin, WI 53132, employees were performing roofing work on an apartment building and were not using fall protection, exposing employees to a fall hazard of approximately 28 feet.  Vulkan Roofing & Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1480067, citation number 2, item number 1 and was affirmed as a final order on October 20, 2020, with respect to a workplace located at 738 Washington St. in Park Ridge, IL 60089.   Vulkan Roofing & Carpentry, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1317751, citation number 1, item number 2 and was affirmed as a final order on November 7, 2018, with respect to a workplace located at Grandview Townhomes 1828 Elizabeth Circle in Sycamore, IL 60178.

29 CFR  1910.178(l)(1)(i):The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  (a) On or about April 26, 2023, at the jobsite located at 9501 W Loomis Road, Franklin, WI 53132, an employee operated a Genie GTH-1056 off-road forklift without being trained in safe operation of the forklift. This exposed employees to possible crushing hazards while moving  materials and equipment onto the roof.
