29 CFR  1910.37(b)(4):Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a.) At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 3/28/23, the employer exposed employees to fire and smoke inhalation hazards, in that emergency evacuation signage along the warehouse was not utilized and provided for employees while performing warehouse shipping and receiving operations.

29 CFR  1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed:  a.) At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 3/28/23, the employer exposed employees to struck by hazards, with an employee traveling with a reach truck containing a palletized load, risen approximately 1 foot in the air obstructing their front view, while performing shipping operations.  b.) At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 4/4/23, the employer exposed employees to struck by hazards, in that an employee traveled with a reach truck with an obstructed front view too close to the shelving unit end cap while another employee performed shelving unit operations with the use of an order picker in mid-air, approximately 12 feet in height.

29 CFR  1910.178(l)(4)(ii):(A) The employer did not provide refreasher training to all employees engaged in the unsafe operation and use of forklifts:  a.) At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 3/28/23, the employer exposed employees to struck by hazards, in that refresher training was not given to a reach truck operators that were recently involved in a collision with a staged palletized load of beverages.   b.)At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 4/4/23, the employer exposed employees to struck by hazards, in that refresher training was not given to a reach truck operators that were recently involved in a collision with another employee performing shelving unit operations with the use of an order picker.
