At the jobsite, located at 6031 Madison Ave Tampa, FL 33619; on or about 4/4/23, the employer exposed employees to struck by hazards, in that an employee traveled with a reach truck with an obstructed front view too close to the shelving unit end cap while another employee performed shelving unit operations with the use of an order picker in mid-air, approximately 12 feet in height.
