29 CFR  1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a.  On or about March 30, 2023, at the above addressed jobsite, employees performing carpentry operations from carpenter scaffolds were exposed to fall hazards due to the scaffold platforms not being at least 18 inches wide.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about March 30, 2023, at the above addressed jobsite, an employee performing residential carpentry operations, eleven (11) feet eight (8) inches above ground, was not utilizing a complete personal fall arrest system, thereby exposing the employee to fall hazards.  MH Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1460596, citation number 2, item number 1, and was affirmed as a final order on April 2, 2020, with respect to a workplace located at 420 Woodchase Ln, Lot/Homesite 10, Vernon Hills, Illinois, 60061.  MH Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1393966, citation number 1, item number 1, and was affirmed as a final order on July 29, 2019, with respect to a workplace located at 444 Woodland Chase, Vernon Hills, Illinois, 60061.

MH Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1460596, citation number 1, item number 3, and was affirmed as a final order on April 2, 2020, with respect to a workplace located at 420 Woodchase Ln, Lot/Homesite 10, Vernon Hills, Illinois, 60061.
