29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about April 21, 2025, employees were exposed to eye injuries while engaged in residential construction activities operating a pneumatic nail gun and a battery powered circular saw. Eye protection was not being utilized while employees engaged in framing activities.

29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as provided in this section.   (a): On or about April 21, 2025, The employer did not ensure each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports as required.  Employees operating at approximately 7.4 feet or more above a lower level were not protected from falling to that lower level.

29 CFR  1926.502(d)(6): Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged: 29 CFR  1926.502(d)(6)(i): directly to webbing, rope or wire rope.   On or about April 21, 2025, the employer did not ensure that employees properly utilized lanyards as part of a personal fall arrest system during residential framing activities. Employees secured the lanyard rope by wrapping the rope around a roof truss of a newly constructed residential structure and attaching it back to itself, thereby subjecting the lanyard to potential friction and damage.

29 CFR  1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.   (a): On or about April 21, 2025, employees utilized a portable ladder to access a scaffold. The unsecured ladder was used on surfaces that were not stable and level, thereby exposing employees to fall hazards.

29 CFR  1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.   (a): On or about April 21, 2025, The employer did not ensure each platform on all working levels of scaffolds was at least 18 inches wide.  Employees operating at approximately 7.4 feet or more above a lower level were operating on a platform that was approximately 10 inches wide.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about April 21, 2025, employees were exposed to falls of approximately 20.0 feet or more above ground level while engaged in residential construction activities. Fall protection was not being utilized while conducting residential roofing activities.  MH Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1660264, citation number 2, item number 1, and was affirmed as a final order on May 26, 2023, with respect to a workplace located at 17062 Foxtail Drive, Orland Park, IL 60467.

29 CFR  1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  On or about April 21, 2025, employees were exposed to fall hazards while performing roofing work and the employer did not certify that employees had been trained to recognize fall hazards, and the procedure to be followed in order to minimize those hazards.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
