29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, so far as possible, a dry condition.  Where wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided.    On March 30, 2023, AGXplore International LLC did not maintain the floor under and around the mixing tanks in a dry condition.  The employees were exposed to slip, trip, and fall hazards.

29 CFR 1910.110(h)(6)(ii)(b): LP Gas Containers shall be protected by crash rails or guards to prevent physical damage unless they are so protected by virtue of their location.  Vehicles shall not be serviced within 10 feet of containers.   On March 30, 2023, AGXplore International LLC had a 500 gallon propane tank placed on the Northside of the warehouse building.  The propane tank was not guarded or protected from vehicular traffic exposing employees in the area to fire and explosion hazards.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  On March 30, 2023, AgXplore International LLC did not verify, through a written certification that workplace hazards assessments had been performed. Employees working at the facilities located at 7035 State Highway D in Parma, Missouri were exposed to hazards such as but not limited to fire, explosion, amputation, crushing, and chemical hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  On March 30, 2023 and times prior to,  AGXplore International LLC employees used compressed air to blow off pallets before loading into tractor trailers.  The compressed air was used for cleaning purposes to blow off dirt and rocks and was not reduced to 30 PSI. Compressed air wands operating at shop pressure (90 PSI) were not equipped with safety nozzles exposing employees to an embolism hazard.
