29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about April 4, 2023, at Cactus Rig # 172 drill site, a red extension cord running through the Company Man's trailer was being pinched by a metal door. This condition exposed employees to electrical shock hazards.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating. On or about April 4, 2023, at the Cactus Rig # 172 drill site, a damaged electrical receptacle located outside the mud engineer's house exposed employees to an electrical shock hazard.
