29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Hensa Stmp, Inc., Welding department hose reel area, on or about January19th, 2023, and times prior thereto: A Fanuc Robot Arc Mate 120 iB robotic welder (Serial # E05503310) was utilized in the hose reel department to perform welding operations. A barrier guard or similar was not installed to prevent employees from accessing the area where the robotic welder was operating potentially exposing employees to struck by hazards. b.) Hensa Stmp, Inc. Welding department-Tower department, on or about January19th, 2023, and times prior thereto: A Fanuc Robot Arc Mate 120 iC robotic welder (Serial #R08700866/E08700345) was utilized in the Tower department welding area to perform welding operations. A barrier guard or similar was not installed to prevent employees from accessing the area where the robotic welder was operating potentially exposing employees to struck by hazards.

29 CFR 1910.147(c)(7)(iv):The employer did not certify that employee training had been accomplished and kept up to date: a) Hensa Stmp, Inc., various production areas, on or about January 19th, 2023 and times prior thereto: Employees were conducting service and maintenance on various pieces of equipment throughout the facility. The employer did not maintain certification of the Lock out Tagout training for authorized employees. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
