29 CFR  1910.303(b)(1):Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:  29 CFR  1910.303(b)(1)(iv): Electrical insulation;   a. On or about March 4, 2023, the employer failed to remove a space heater from service that had a exposed damaged cord, thereby exposing employees to electrical shock and/or fire hazards.

29 CFR  1910.303(b)(2):Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a. On or about March 4, 2023, two 120 VAC extension cords were daisy chained together to energize a space heater in the press area. The cord was being utilized outside its approved use.

29 CFR  1910.303(g)(2)(i): Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means:  a. On or about March 4, 2023, the employer had exposed 120VAC electrical wires located under the electrical disconnects in the agitator area, thereby exposing employees to electrical shock hazards.  b. On or about March 4, 2023, the employer had exposed 120VAC electrical wires dangling from a conduit approximately 5 feet from the floor attached to the overhead mezzanine in the clean out area below the presses, thereby exposing employees to electrical shock hazards.

On or about March 4, 2023, two extension cords were being used to provide 120 VAC to a space heater located in the press area. The cords were being used in lieu of fixed wiring, thereby exposing employees to electrical shock and/or fire hazards.
