On or about April 4, 2023 employees were exposed to a struck by hazard while performing sewer line replacement at a residential home located at 31821 Center ridge Road, Olmsted Falls Ohio 44138.  An employee was working in the trench and the employer did not ensure all tools were set 2 feet back from the edge.  Leach Excavating LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.651(j)(2), which was contained in OSHA inspection number 1606312, citation number 1,  item number 2 and was affirmed as a final order on 9/6/22, with respect to a workplace located at Rockside Road and Skyline Drive, Seven Hills, Ohio 44131.
