29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) Phase 1 -2 Hovtech Boulevard, Mount Laurel NJ: On or about 04/06/23 two company workers were exposed to fall hazards, up to and in excess of 25 feet to the ground surface below, when working and walking from a job made supported scaffold that was not supplied with guard rails and whose walking/working surface was not fully planked. At the time of their exposure to the fall hazards, one worker was NOT using fall protection and the other worker was improperly using fall protection that contained visible defects and was improperly installed. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) a - Phase 1 -2 Hovtech Boulevard, Mount Laurel NJ: On or about 04/06/23 two company workers were exposed to fall hazards, up to and in excess of 25 feet to the ground surface below, when working and walking from a job made supported scaffold that was not supplied with guard rails and whose walking/working surface was not fully planked. At the time of their exposure to the fall hazards, one worker was NOT using fall protection and the other worker was improperly using fall protection that contained visible defects and was improperly installed. Through statements provided to the CSHO, the company worksite workers and Foreman had not received fall protection training from the company as specified in 29 CFR 1926.503 (a)(2)(i) through (viii). NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
