On the roof of a temporary commercial worksite, located at 2550 Gilmore Street (Central Riverside Elementary School) Jacksonville, FL 32205: On or about April 7, 2023, the employer Thomas May Construction Company failed to ensure that its sub-contractor D. R. Roof, Inc. protected its employees from eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.
