29 CFR 1910.178(l)(2)(ii):  The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:     At the worksite located at 1248 Crystal Avenue, Kansas City, Missouri:  Employees were exposed to struck by hazards from powered industrial trucks.  Powered industrial truck operators were not presented training on forklifts in either video form or classroom lecture/discussion form.  Powered industrial truck operators have not been evaluated by the employer in use of forklifts.

In the welding area, electrical breaker panels were blocked by shelving, tools, and materials.    2.  In the manufacturing area, live electrical breaker panels were blocked by shelving, tools, and materials such as wooden boards and buckets of compound. 3.  In the manufacturing area, live electrical breaker panels were blocked by scaffolding and wooden boards.

On the west wall in the manufacturing area, electrical equipment operating at 50 volts, or more was not enclosed to protect from accidental contact due to the electrical cabinet door being left open.   2.  On the north wall in the manufacturing area, electrical equipment operating at 50 volts, or more was not enclosed to protect from accidental contact due to the electrical cabinet door being left open.
