29 CFR  1910.147(c)(4)(i):Energy control procedure. Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  a) On or about April 11, 2023 in building 2735, the employer did not utilize a hazardous energy control procedure located in the share drive for equipment such as but not limited to the Sicoma Concrete Mixer in that the procedures were not utilized to lock out energy sources such as but not limited to the electrical disconnect prior to cleaning.  b) On or about April 19, 2023 in building 123, the employer did not follow the hazardous energy control procedure for equipment such as but not limited to the Hurco VMX64Ti mill while cleaning the equipment in that a lock was not applied to the air valve located on the mill.

29 CFR 1910.147(c)(8):Energy isolation. Lockout or tagout shall be performed only by the authorized employees who are performing the servicing or maintenance.  a) On or about May 24, 2023 in building 2735 Crane Army Ammunition Activity did not ensure each authorized employee conducting cleaning the Sicoma Concrete Mixer performed lockout in that only one authorized employees  applied the lock to the electrical disconnect for a total of 2 authorized employees.  b) On or about April 19, 2023, in building 123 the employer did not ensure each authorized employee changing the coolant and cleaning the Hurco VMTX65Ti mill performed lockout in that only the supervisor locked out the electrical disconnect.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (d)(1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy.  (d)(2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.  (d)(3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  (d)(4) Lockout or tagout device application.  (d)(4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  (4)(ii) Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position.  (d)(6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.  a) On or about April 11, 2023 in building 2735 each authorized employee performing cleaning on the Sicoma Concrete Mixer did not perform the established procedures for (d)(1) through (d)(4) and (d)(6) prior to reaching into the concrete mixer.

Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) On or about April 11, 2023, Crane Army Ammunition Activity did not adequately train authorized employees located in buildings 123 and 2735 in the recognition of applicable hazardous energy control procedures and energy sources, and the methods and means for energy isolation and control in that authorized employees were not isolating the electrical disconnect while cleaning the Sicoma Concrete Mixer and the air supply of the Hurco VMX64Ti while cleaning and changing the coolant.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) On or about April 11, 2023, in building 123 the employer did not adequately guard from rotating parts on the scrap metal conveyor installed onto the #4 Okuma LU25M serial number 0318.   b) On or about April 11, 2023, in building 123 the employer did not adequately guard from rotating parts on the scrap metal conveyor installed onto the  Okuma LB3000EXII serial number 228228 CNC lathe.   c) On or about April 26, 2023, in building 123 the employer did not adequately guard employees from the rotating parts on the scrap metal conveyor installed onto the #5 HAAS DS-30Y serial number 3117224 lathe.   d) On or about April 26, 2023, in building 123 the employer did not adequately guard employees from the rotating parts on the scrap metal conveyor installed onto the #6 Okuma LB3000EXII serial number 228236 CNC lathe.   e) On or about April 26, 2023, in building 123 the employer did not adequately guard the exposure caused by the pinch point caused by the opening and closing of the finished part discharge door on the HAAS ST-30SS serial number 3090019.  f) On or about April 26, 2023, in building 123 the employer did not adequately guard employees from the exposure of the point of operation of the TSUNE TKA50GL serial number 01832 .  g) On or about April 26, 2023, in building 123the employer did not adequately guard employees from exposure of the point of operation of the Kent USA KTM-3VKF serial number 9911979 drill press.  h) On or about April 11, 2023, in building 2735 the employer did not adequately guard the point of operation of the Station 2 Jet TCA-70 which operated at 1,486 revolutions per minute in that employees who operated the drill press were exposed to nip points and rotating parts.   i) On or about April 11, 2023, in building 2735 the employer did not adequately guard employees from the point of operation of the Station 1 Jet TCA-70 serial number 171086 which operated at 553 revolutions per minute in that employees who operated the drill press were exposed to nip points and rotating parts.  j) On or about May 23, 2023, in building 3331 the employer did not have adequate guarding on a rectangular notcher for a Scotchman Ironworker DO70/110-24M (SN:1448M0910) exposing employees currently operating the machine to caught in hazards.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.  a) On or about April 26, 2023, Crane Army Ammunition Activity did not reduce air used for cleaning in that employees used the approximately 65 p.s.i. air nozzle while cleaning the table of the AKS Cutting Systems WKX3T-612-14 water jet.    b) On or about April 26, 2023, Crane Army Ammunition Activity did not reduce compressed air used for cleaning in that employees used the approximately 60 p.s.i. air nozzle while cleaning metal chips on the #19 HAAS TL2 serial number 3105770.  c) On or about April 26, 2023, Crane Army Ammunition Activity did not reduce compressed air used for cleaning in that employees used the approximately 60 p.s.i. air nozzle while cleaning metal chips on the #20 HAAS TL2 serial number 3113172.  d) On or about April 26, 2023, Crane Army Ammunition Activity did not reduce compressed air used for cleaning in that employees used the approximately 60 p.s.i. air nozzle while cleaning metal chips on the #21 HAAS TL2 serial number 3110094.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a) On or about April 25, 2023, in Building 3205 a hanger door next to the fire door blocks the fire exit from opening when the hanger door was opened.
