Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about 10 April 2023, an employee for Cheltenham Construction Services, Inc. dba CCS was exposed to a fall hazard at 521 Greengage Timber Drive in O'Fallon, Missouri. The employee was installing approximately 32 square feet of siding on a portion of the second story of the residential building. The work location required the employee to straddle the valley of two roof slopes one being approximately a 3:12 slope and the other being approximately an 10:12 slope. The employee was not equipped with a fall protection system. This condition exposed the employee to a fall height of at least 9 feet over the concrete driveway.  Cheltenham Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1556648, citation number 1, item number 1 and was affirmed as a final order on 3 March 2022, with respect to a workplace located at 713 Alban Alley, O'Fallon, Missouri.  Cheltenham Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1455845, citation number 2, item number 1 and was affirmed as a final order on 19 February 2020, with respect to a workplace located at Old Park Lane, Swansea, Illinois.
