29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On April 11, 2023, employee(s) worked from jack scaffolds without fall protection systems while installing cornice material on the exterior of a residential home.  The employee(s) were exposed to a fall hazard of approximately 15 feet to the ground below.  a)  Employee working on the west side of the home from a jack scaffold without fall protection, b)  Employee working on the south side of the home from a jack scaffold without fall protection, d)  Employee working on the east side of the home from a jack scaffold without fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On April 11, 2023, employees using an extension ladder that did not have the side rails extended three feet over the upper landing surface were exposed to potential fall hazards greater than ten feet.
