29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. On or about April 11, 2023, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about April 11, 2023, at the above addressed jobsite, an employee performing siding activities from a pump jack scaffold, thirteen (13) feet nine (9) inches above the ground, was exposed to injuries from falls due to the scaffold not being fully guarded on all open sides.
