29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) At the facility, 1000 E. Union Street, Newark, NY. On or about April 12, 2023, employees were performing servicing activities including, but not limited to setup and die/cutter changes on machines, including, but not limited to the Accurshear, Ironworker, Metal Muncher and Press Brake, which have multiple energy sources, including electric, pneumatic, and hydraulic on a weekly basis. The machines have multiple energy sources including but not limited to electric, pneumatic, nd hydraulic. The employer did not establish a program consisting of energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy and rendered inoperative.
