29 CFR 1926.503(a)(2)(iv):The employer did not ensure that each employee was trained, as necessary by a competent person in the role of each employee in the safety monitoring system where the system was used: On or about April 12, employees working on a commercial flat roof were exposed to fall hazards, while the safety monitor was engaged in activities that took their attention away from the monitoring function, and the employer had not ensured that each employee had been adequately trained in their roles while utilizing a safety monitoring system.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about April 12, 2023 employees performing roofing activities were exposed to fall hazards greater than 25 feet and the employer did not ensure that appropriate fall protection was utilized. The Jacksonville Insulation & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1358556, citation number 1, item number 2-a and was affirmed as a final order on December 4, 2018, with respect to a workplace located at 426 S. Main St., Jacksonville, IL 62650.
