29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about March 29, 2023, where an employee was exposed to a crushed-by hazard when working in an excavation without it being protected from cave-ins by an adequate protection system. ISJ Underground Utilities, LLC. was previously cited for a violation of the equivalent occupational safety and health standard 29 CFR 1926.652(c), which was contained in OSHA inspection number 1658189, citation number 1, item number 3 and was affirmed as a final order on May 31, 2023, with respect to a workplace located at 7506 Cypress Spring Road Spring, Texas 77379.
