29 CFR 1910.133(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable. a) Woodshop: On or about April 12, 2023, an employee was planning wood planks without side eye protection clipped onto their prescription eyeglasses. This exposed employees to an eye hazard from flying debris created by the planner.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. a) Warehouse: On or about April 12, 2023, outside the warehouse entrance there were two 55-gallon barrels and one 35-gallon barrel stored in front of two 600 VAC heavy duty safety switches. This condition exposed employees to an arc flash, shock hazard and delayed emergency access.
