On or about April 4, 2023, the employer did not ensure the Clifford Press No. 3 operator(s) performing service and maintenance activities on the Clifford Press No. 3, utilized the developed and documented energy control procedures to control the hazardous energy sources (electrical, pneumatic, hydraulic, water, gravity), while unjamming a metal part (panel) from the Clifford Press No. 3 nibber (shear) equipment.

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about April 4, 2023, the employer did not ensure the Clifford Press No. 3 operator(s) performing service and maintenance activities by unjamming a metal part (panel) on the Clifford Press No. 3 applied and followed the established energy control procedures by; turning off or shutting down the Clifford Press machine [per 1910.147(d)(2)], isolating the Clifford Press machine from the electrical, pneumatic, hydraulic, water, and gravity energy sources [(d)(3)], affixing lockout device(s) to the energy isolation device(s) [(d)(4)(i), and verifying de-energization and isolation had been accomplished [(d)(6)], prior to unjamming the metal part (panel) from the machine.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about April 4, 2023, the employer did not ensure one or more methods of machine guarding was provided to protect the operator and/or assistant operator from hazards created by the point of operation of the resistant welder press and ingoing nip points and moving parts of the nibber (shear) of the Clifford Press No. 3.
