29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area. a) Press Department, Press # 8, On or about April 6, 2023, when the machine was in jog mode while a maintenance employee was validating repairs, the work area was not checked to ensure employees were in safe position prior to jogging the machine. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

Operators were not trained with the understanding, knowledge, and skills necessary to be authorized employees or provided with adequate instruction to understand the function of affected employees. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
