29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about April 5, 2023, two employees were climbing over and working from a concrete form above the protective railing installed below it, exposing the employees to a fall hazard.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about April 5, 2023, three employees were accessing the top of and the inside of a four-foot-high wooden concrete form without the use of a ladder, exposing the employees to a fall hazard.
