29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation: Located at Hank's Furniture Warehouse at 11250 Smitty Lane, North Little Rock, AR 72117: On or about April 17, 2023, the OSHA 300 log for the calendar year 2023 was not completed in the detail required in that: a) On April 13, 2023, Order Picker - fell approximately 16-feet to the concrete floor, landing on both feet and sustained fractures to both ankles. The description of the injury did not include the object/substance that directly injured the employee. b) On April 15, 2023, Warehouse Worker - An injury resulted in 10 days away from work. The description did not include the injury sustained, the object/substance that directly injured or made the employee ill.

Located at Hank's Furniture Warehouse at 11250 Smitty Lane, North Little Rock, AR 72117: On or about April 17, 2023, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2023: a) On April 13, 2023, Order Picker - fell approximately 16-feet to the concrete floor, landing on both feet and sustained fractures to both ankles. The Workers Compensation - First Report of Injury or Illness form used was not filled out in its entirety and was missing the required information.
