29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m):   The employer does not ensure compress gas cylinders of oxygen and acetylene were stored at least 20' apart, exposing employees to fire and explosion hazards.      a)	This violation was observed on or about April 17, 2023, where compressed gas cylinders of oxygen and acetylene were observed with gauges still on and according to employees, not used within 24 hours.  Cylinders located on east wall of facility in middle of shop.  b)	This violation was observed on or about April 17, 2023, where compressed gas cylinders of oxygen and propane were observed with gauges still on and according to employees, not used within 24 hours. A small cylinder of acetylene was also located with the other cylinder. Cylinders located on east wall of facility in middle of shop.  c)	This violation was observed on or about April 17, 2023, where compressed gas cylinders of oxygen and acetylene were observed with gauges still on and according to employees, not used within 24 hours. The pendant box was located on the west wall in the facility, exposing employees to electrical hazards.  Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure that unused openings in cabinets, boxes, and fittings are effectively closed.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  The employer does not ensure working space is maintained around 600-volt panels by allowing product and other materials to be stored in front of the panels, exposing employees to shock hazards.  a)  This violation was observed on or about April 17, 2023, where a 480-volt panel was blocked by a double stacked toolbox.  The panel located on the north side of the shop.   b)  This violation was observed on or about April 17, 2023, where a 480-volt panel was blocked by product and other equipment in the shop.  The panel located on the north side of the shop. The flexible cord used as permanent wiring was observed in a paint booth along the west wall in the middle of the facility.    b)  This hazard was observed on or about April 17, 2023, where an extension cord was wrapped around beams and other parts of the structure.  The flexible cord used as permanent wiring was observed in a paint booth along the east wall in the middle of the facility.    c)  This hazard was observed on or about April 17, 2023, where an extension cord was wrapped around beams and other parts of the structure.
