On or about 12 April 2023, one employee of CM Exteriors LLC was observed working on a scaffold to put up siding on a residential home located at Lot 100 Amberleigh Manor Drive Wentzville, MO. Part of this scaffold had overlapped platforms to create a long platform and this overlap did not occur over a support.

29 CFR  1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed with force so as to distort the scaffold's structural integrity:  On or about 12 April 2023, one employee of CM Exteriors LLC was observed working on a scaffold to put up siding on a residential home located at Lot 100 Amberleigh Manor Drive Wentzville, MO. The fabricated half of the scaffold relied on an A-frame step ladder for support. This ladder did not have a metal spreader or locking device to lock the device in position while the device was in use, thus affecting the scaffolds structural integrity.

On or about 12 April 2023, one employee of CM Exteriors LLC was observed working on a scaffold to put up siding on a residential home located at Lot 100 Amberleigh Manor Drive Wentzville, MO. The scaffold on which work was being completed did not have firm foundation support of base plates and mudsills.

The ladder used to support the fabricated portion of the scaffold was an unsecured A-frame ladder on a n approximately 5/12 roof.

The area around the top of the extension ladder at the front of the house was not kept clear of debris. The area around the bottom of the access ladder at the rear of the house was surrounded by debris.
