OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. Ohio Steel Processing, LLC dba Ohio Pickling & Processing located at 1149 Campbell St. Toledo, Ohio: On or about April 18, 2023, employees tasked with operation of the CRB 35 Ton Cab Controlled Bridge Crane S/N 7031706L equipped with a below-the-hook lifting device that did not maintain a rated load capacity label, were exposed to struck by and crushing hazards while lifting and moving large coiled steel.

29 CFR 1910.305(b)(3)(i):Boxes did not provide a complete enclosure for the contained conductors or cables. a. Ohio Steel Processing, LLC dba Ohio Pickling & Processing at the worksite of 1149 Campbell St., Toledo, Ohio: On or about April 18, 2023, the employer failed to ensure that an electrical box located at the steering unit of the Pickling Line was provided with a sufficient cover. Employees operating in proximity of exposed live conductors were exposed to electrical shock and electrocution hazards.
