29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   a) RAS Process Equipment Inc. - Manufacturing and yard areas:  Fork extensions were used on the Mitsubishi Propane Forklift Model # FG-35, S/N 100L-SS-B093 to move material and product throughout the production plant, Written approval for the use of the fork extensions was not obtained from the forklift manufacturer, on or about 04/18/2023.
