29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level. Location: 646 Ocean Avenue, Revere, MA On or about 4/18/2023, one worker fell 9 feet from an upper surface, impacting the lower level while wearing a personal fall protection system that was not configured correctly. In the configuration used there was no calculation for swing distance, freefall distance, deceleration distance, harness stretch, and safety factor as illustrated in the manufacturer's instructions.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. Location: 646 Ocean Avenue, Revere, MA On or about 4/18/2023, one worker fell 9 feet from an upper surface, impacting the lower level while wearing a personal fall protection system that was not configured correctly. The employee had general safety training and several of the onsite supervision had specific fall protection training however the PFAS anchorage points were not set per manufacturer guidelines, lifeline slack calculations were not made per manufacturer guidelines, and the layout of the site overall made effective implementation of the chosen fall protection system ineffective.
