29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. Location: Worksite On about July 1, 2025 employees were exposed to equipment struck-by hazards while working in and around an aerial that had not been inspected prior to its use. The aerial lift's audible alarm was not operational. ANSI A92.24-2018 Equipment Inspections. The standard requires inspections to be performed daily prior use. To include Audible and Visual Alarms and Beacons.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold (including aerial lifts) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Location: Worksite On or about July 1, 2025, the employer did not assure that the worker operation a boom-lift had been trained in its operations.
