29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about April 18, 2023, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about April 18, 2023, at the above addressed jobsite, an employee performing carpentry operations on the outside top plate, eleven (11) feet six (6) inches above the ground, was not protected from falling, thereby exposing the employee to fall hazards.  b. On or about April 18, 2023, at the above addressed jobsite, employees performing carpentry operations on the first floor ceiling , approximately nine (9) feet above the lower level, was not protected from falling, thereby exposing the employees to fall hazards.  Blue-Gold Construction, Inc. was previously cited for a violation of this occupational safety and health standard  1926.501(b)(13), which was contained in OSHA inspection number 1353636, citation number 1, item number 1 and was affirmed as a final order on 01-22-2019, with respect to a workplace located at Lot #42 Olde Stone Village, Frankfort, IL.  Blue-Gold Construction, Inc. was previously cited for a violation of this occupational safety and health standard  1926.501(b)(13), which was contained in OSHA inspection number 1577661, citation number 1, item number 1 and was affirmed as a final order on 06-8-2022, with respect to a workplace located at Lot #87 Torino Way, Frankfort, IL.
