29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). On or about April 20, 2023, an employee was allowed to operate a powered industrial fork truck prior to receiving the required training, exposing employees to struck- by hazards. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system: On or about April 20, 2023, employees performing framing operations on the upper level at a single-family property were exposed to a 13.6 foot fall when the employer failed to ensure that fall protection was utilized. C.G.O. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1316461, citation number 1, item number 1 and was affirmed as a final order on August 6, 2018, with respect to a workplace located at 4603 Oakwood Ave. Downers Grove, IL 60515. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
