29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the forklift and the forklift is not moved horizontally while the platform is occupied: On or about January 17, 2025 the employer used a telehandler lift to raise a work platform basket to lift employees in excess of 20 feet high. Employees were observed performing framing operations in an elevated work platform by a Genie GTH-844 that was not attached and secure to the forklift, exposing employees to fall and struck-by hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about January 17, 2025, an employee was exposed to fall hazards in excess of 20 feet while engaged in roof construction activities from the working platform attached to the telehandler. Employer did not ensure the employee was protected by personal fall protection equipment as specified in the telehandler manual. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were exposed to fall hazards in excess of 20 feet while engaged in roofing activities from a work platform attached to the Genie GTH-844 Telehandler and not utilizing fall protection. Fall protection was not being utilized while employees were engaged in installing new residential roofing. C.G.O . Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1664600, citation number 2, item number 1 and was affirmed as a final order on June 30, 2023, with respect to a workplace located at 215 N. Washington, Hinsdale, IL. 60521. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
