On or about April 21, 2023, at the church on 2400 SR 434, Longwood, FL, employer allowed removing old roofing shingles while working on a roof 12 feet high without the use of fall protection, exposing employees to fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a. On or about April 21, 2023, at the worksite located at 1709 Howell branch Road, Winter park, FL, employer allowed the use of their ladder to access the work area that was set approximately 1.5 feet past the roof landing, exposing employees to fall hazards of approximately 12 feet.
