Employees engaged in roofing activities on a 6:12 pitched roof were exposed to fall hazards of approximately 10 feet 7 inches from the eve of the residences to the exterior ground below. Employees were wearing harnesses but were not tied off.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the temporary construction site located at 14230 Hunter's View Wichita, KS, employees were exposed to fall hazards. Employees engaged in roofing activities on a 6:12 pitched roof were exposed to fall hazards of approximately 10 feet 7 inches from the eve of the residence to the exterior ground below. The aluminum extension ladder used to access the roof did not extend 3 feet from the working surface nor was secured to the structure.
