29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: At a worksite located at 27200 E Cedar Grove Dr., Lake Lotawana, Missouri, an employee was exposed to head injury hazards from falling objects. The employer exposed the employee to these hazards by allowing them to work without appropriate head protection while working under employees at a higher level.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: At a worksite located at 27200 E Cedar Grove Dr., Lake Lotawana, MO 64086, employees were exposed to a fall hazard. The employer exposed the employees to this hazard by allowing them to work from a wooden pallet that was supported by a rough terrain forklift. The employees were not protected from falling by any means of fall protection. The employer, Pettengill Family Restoration LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(c)(2)(iv), which was contained in OSHA inspection number 1596526, citation number 1, item number 1 and was affirmed as a final order on 10-12-2022, with respect to a workplace located at Lot #35 E. Sycamore St., Lake Lotawana, MO 64086.
