29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Plant 1, Flour Tortilla Production Department, Line 5 - On or about April 15, 2023, and at times prior to, the employer exposed employees to amputation hazards in that, the employer failed to develop, document, and utilize procedures for the control of potentially hazardous energy of the dough feeder and dough rounder/divider machine when employees were cleaning and removing dough that accumulated in the section between the dough feeder and the dough rounder/divider.
