29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Plant # 4, Flour Line - On or about July 18, 2023, and at times prior to, employees were exposed to amputation and electrical shock hazards, in that the employer failed to ensure that Press Machine # 4 (also referred to as Flour Machine - Model # MP596 & Serial Number # OMP525231) was shut down and deenergized prior to performing servicing and maintenance.
