On or about April 24, 2023, employees removed metal frames 6 feet above the lower level without fall protection.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent scaffold movement while the scaffold was used in a stationary manner. On or about April 24, 2023, employees removed metal frames 6 feet above the lower level using a mobile scaffold where the caster was not locked to prevent movement while in use.
