29 CFR  1910.23(b)(9):Ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury;  (a) On or about April 24, 2023, facility workers were exposed to a fall hazard where the employer failed to inspect rolling ladders for visible defects before use. During the walkaround inspection in B Warehouse there was a damaged Rolling Ladder (MD0615), with yellow caution tape on the ladder.  (b) On or about April 24, 2023, facility workers were exposed to a fall hazard where the employer failed to inspect rolling ladders for visible defects before use. During the walkaround inspection in between A and B Warehouse, missing rubber feet on Uline 10 step ladder were identified.

29 CFR  1910.37(a)(3):Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a)  At the workplace located in the C warehouse was a blocked exit route on the south wall nearest to Culvers by multiple boxes stored on the floor.  b) At the workplace located in the C warehouse near the loading docks was a blocked exit route temporarily, staging palletized material near loading dock exit.

29 CFR  1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a) On or about April 24, 2023 , located in between Warehouse A and B,  the portable fire extinguishers were blocked by racking and mobile carts. The fire extinguisher located on the west wall in between Warehouse A and B, was blocked by a mobile cart.   b) On or about April 24, 2023, located in between Warehouse A and B, the portable fire extinguishers were blocked by racking and mobile carts. The fire extinguisher located on the south wall next to the Culver's exit door was blocked by pallet racking system.

29 CFR  1910.178(g)(2):Facilities shall be provided for flushing and neutralizing spilled electrolyte, for fire protection, for protecting charging apparatus from damage by trucks, and for adequate ventilation for dispersal of fumes from gassing batteries.  a) On or about April 24, 2023, the employer did not protect the battery charging area from forklift damage located in Warehouse B. During the inspection battery charger #6 (SCR Charger 100) had sustained damage to the front access door and had a plastic zip tie to secure the door from opening exposing employees to electrical hazards.

Sufficient access and working space shall be provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.  a) On or about April 24, 2023, located in the A Warehouse Area on the East wall, electrical cabinet Panel H was blocked by racking and storage material, exposing employees to electric shock and fire hazards.   b) On or about April 24, 2023, located in the B Warehouse area, electrical equipment was blocked and used for storage in the following location;  a 480V electrical panel was blocked by pallet racking and crated material, exposing employees to electric shock and fire hazards.

Defects when found shall be immediately reported and corrected.
