29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) Cast House - On or about April 23, 2023 and at times prior thereto, an employee was draining the bowl when there was an explosion due to molten aluminum coming into contact with water that resulted in an employee covered in molten aluminum. The employee received burns to his hand and neck and was not protected from burn hazards.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Cast House - On or about April 25, 2023 and at times prior thereto, the incident that occurred on 4-23-23 at approximately 7:30am was not reported by the employer until 4-24-23 at 4:15pm, approximately 33 hours after the incident.
